Family Support Worker

The successful candidate will be responsible for undertaking Child and Family Assessments, S.47 inquiries, and intervention planning for 'Children in Need'. They will assess, monitor, and review social care needs of individuals, relatives, and carers, arranging care services as necessary. Additionally, they will develop individual care plans based on needs and risks, taking action to protect vulnerable people. They will manage an agreed caseload, offering support and guidance, and monitoring and reassessing care packages regularly.
