We are seeking a dedicated and compassionate Reablement Care and Support Worker to join our team in North Yorkshire. As a key member of our Reablement team, you will play a vital role in supporting individuals to regain and maximize their independence in their own homes.

Key Responsibilities

  • Provide hands-on care and support to vulnerable adults in their own homes, promoting their independence and well-being.
  • Assess individual needs and develop personalized support plans to meet their unique requirements.
  • Work collaboratively with colleagues and external agencies to ensure seamless delivery of care and support services.
  • Contribute to the development and implementation of new initiatives and programs to enhance the quality of life for our service users.

Requirements

  • Level 2 qualification in Health and Social Care (or willingness to undertake training and achieve this qualification).
  • Ability to travel across the region to work in the homes of adults with care and support needs.
  • Flexibility to work a variety of shifts, including evenings and weekends, as part of a 7-day service.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, colleagues, and external partners.
