Job Title: Community Heart Failure Specialist Nurse

This is an exciting opportunity for a highly motivated and experienced nurse to join our community-based team of Heart Failure Specialist Nurses delivering advanced practice care to patients with chronic heart failure in the Harrogate and District NHS Foundation Trust area.

Main Responsibilities:
  • Manage an identified group of patients with complex health and social care needs, using a case management model.
  • Combine highly specialist nursing knowledge and advanced skills to proactively manage patients with Heart Failure, improving the quality, experience, and choice about their care, enabling them to remain at home longer.
