About the Role

This exciting opportunity has arisen for a skilled social worker to join our Hospital Discharge team in Cardiff. As a key member of our team, you will play a vital role in ensuring the smooth transition of patients from hospital to community settings, prioritizing timely discharges, and working collaboratively with healthcare professionals.

Main Responsibilities:

  1. Collaborate with multidisciplinary teams to coordinate patient transfers and ensure seamless transitions
  2. Develop effective care plans that address individual needs and preferences
  3. Communicate regularly with patients, families, and caregivers to provide updates and support
  4. Conduct thorough assessments to identify potential barriers to discharge
  5. Work closely with colleagues to prioritize cases and allocate resources effectively

Your Qualifications and Skills:

">
  • Minimum 6 months experience as a registered social worker in the UK
  • Relevant social work qualification and registration with either Social Care Wales or Social Work England
  • Excellent communication and interpersonal skills
  • Ability to work under pressure and manage competing priorities
  • Knowledge of relevant legislation and policies governing adult social care
