**About the Role**

Hoop Social Work is recruiting a highly skilled Registered Social Worker for an initial 3-6 month contract to support with Winter pressures. This position offers a fantastic opportunity to work in a fast-paced environment affecting timely discharges from hospital.

**Responsibilities**

  • Discharge planning and coordination
  • Effective time management and prioritisation skills
  • Ability to chair meetings and communicate with various professionals
  • Excellent assessment skills and knowledge of relevant legislation

**What You Need**

  • A minimum of 6 months experience as a registered social worker in the UK
  • A relevant Social Work Qualification
  • Registration with Social Care Wales / Social Work England
  • Enhanced DBS registration on the update service or willingness to apply for one
  • 3 years of professional referencing
  • Driving license with access to a car
