**Job Summary:**

We are seeking a skilled Social Worker to join our hospital discharge team in Cardiff. The successful candidate will be responsible for ensuring timely discharges from three hospitals: UHW, Llandough, and St Davids.

The ideal candidate will have excellent assessment skills, the ability to work effectively in a team, and sound knowledge of relevant legislation.

**Key Responsibilities:**

  • Carry out discharges across three hospital sites
  • Work with a wide range of professionals to ensure effective discharge planning
  • Ensure that the voice of citizens is championed at all times

**Requirements:**

  • A minimum of 6 months' experience as a registered social worker in the UK
  • A relevant Social Work Qualification
  • Registration with Social Care Wales or Social Work England
  • Enhanced DBS and access to a car

**Salary:** Up to £37 per hour payrolled via an umbrella company

**Location:** Cardiff, South Wales
