Discharge Planning Specialist

We are seeking a highly skilled Social Worker to join our Hospital Discharge team in Cardiff. As a Social Worker, you will be responsible for carrying out discharges across 3 hospital sites. The successful candidate will have excellent time management skills, prioritization skills, and the ability to effectively chair meetings.
The ideal candidate will have a minimum of 6 months experience of working in the UK as a registered social worker and hold a relevant Social Work Qualification. Registration with Social Care Wales / Social Work England is also required.
This is a fantastic opportunity to work in a fast-paced environment to affect timely discharges from hospital. You will be working in an experienced and supportive team to champion the voice of the citizen at all times.
The role offers a competitive salary of up to £37 per hour payrolled via an umbrella company, dependent upon experience and references. Additionally, you will receive a range of benefits including PAYE and Umbrella payroll options available – you choose, dedicated point of contact, twice weekly payroll, and free on-site quarterly training sessions.
