About the Role

We are seeking a Support Practitioner to join our team in Midlothian, where you will work closely with service users to identify their needs and develop tailored support plans. Your primary responsibility will be to provide person-centered support, focusing on promoting independence, dignity, and overall well-being.

Your day-to-day activities may involve supporting service users with daily tasks, such as personal care, medication management, and household chores. You will also facilitate access to community resources, help with goal setting, and maintain accurate records of service user progress.

To succeed in this role, you should possess excellent communication and interpersonal skills, with the ability to build trust and rapport with service users, colleagues, and external agencies. A strong commitment to delivering high-quality support is essential, as well as a willingness to learn and adapt to changing circumstances.

Salary: £23,000 - £25,500 per annum (dependent on experience)
