Transform Lives as a SCPHN School Nurse

We are seeking a talented and compassionate SCPHN School Nurse to join our team in Central Lancashire on a full-time basis. As a valued member of our team, you will have the opportunity to make a positive impact in the lives of children and young people by promoting health, wellbeing, and providing essential care.

Working within Lancashire Child and Family Wellbeing Services offers a unique environment with clear visibility of senior management who are approachable due to their hands-on approach. You will feel valued and listened to, with opportunities to have your voice and ideas heard.

Main Responsibilities:
  • Deliver the Healthy Child Programme, assessing health needs, developing, implementing, and evaluating packages of care.
  • Use the Family Health Needs Assessment tool to plan and evidence care.
  • Develop and implement individualized care plans for each child based on a needs assessment, involving the child/parent/carer/family in planning and implementation.
  • Participate in effective partnership working with other agencies and take a lead professional role where appropriate.
  • Apply evidence-based practice to develop and maintain a high-quality and cost-effective standard for Specialist Public Health Nurse/School Nursing practice.
