We are seeking a skilled Health and Wellbeing Coach to support patients with long-term conditions in managing their health and wellbeing. As a key member of our team, you will empower patients to understand and manage their conditions independently using health coaching techniques.
Main Responsibilities- Provide personalized care and wellbeing coaching to a cohort of patients.
- Work closely with GPs and multidisciplinary teams to develop and implement patient care plans.
- Participate in multidisciplinary team meetings to discuss patient progress.
- Engage with community resources and initiatives to promote healthy lifestyles.
- Manage a caseload independently, ensuring patient outcomes are met.
- Support quality improvement projects across the PCN.
