About the Role

We are seeking a skilled and compassionate Health and Wellbeing Practitioner to join our team at Herefordshire and Worcestershire Health and Care NHS Trust. As a key member of our Worcester Neighbourhood Mental Health Team, you will play a vital role in improving the mental health and wellbeing of our local communities.

Key Responsibilities
  • Provide prompt local assessment, triage, risk assessment, risk management, signposting, onward referral, and evidence-based interventions for people presenting with mental health problems.
  • Work across all clinically appropriate patients, 17 years old and over, to deliver evidence-based interventions.
  • Represent the team on specific projects and working groups as required.
Person Specification
  • Knowledge of principles of assessment of people with mental health problems.
  • Knowledge of principles of risk assessment and management.
  • Knowledge of Mental Health Act 1983 and Mental Capacity Act 2006 and other relevant legislation.
Requirements
  • Relevant professional qualification relating to Health or Social Care e.g. in social work, nursing, OT with current professional registration.
  • Recent evidence of continuing professional development.
