Job Summary

We are seeking a skilled Apprentice Health and Wellbeing Practitioner to join our team at Herefordshire and Worcestershire Health and Care. As a key member of our Community Mental Health Service, you will work closely with patients, healthcare professionals, and other stakeholders to deliver high-quality interventions and support.

Key Responsibilities
  • Provide evidence-based interventions to patients aged 17 and over, working across all clinically appropriate areas.
  • Assess, triage, and manage risk, as well as signpost and refer patients to relevant services.
  • Represent the team on specific projects and working groups, contributing to the development of our services.
Requirements

To succeed in this role, you will need to demonstrate strong communication and interpersonal skills, as well as the ability to work independently and as part of a team. You should be able to think critically and creatively, with a strong commitment to delivering high-quality patient care.
