About the Role

We are seeking a skilled and compassionate Health and Wellbeing Practitioner to join our team at Herefordshire and Worcestershire Health and Care NHS Trust. As a key member of our Community Mental Health Service, you will play a vital role in delivering high-quality care to patients across Worcester.

As a Health and Wellbeing Practitioner, you will work closely with our multidisciplinary team to provide prompt local assessment, triage, risk assessment, risk management, signposting, onward referral, and evidence-based interventions for people presenting with mental health problems.

You will have the opportunity to design and deliver evidence-based recovery-focused mental health interventions and groups, working with patients to develop personalized care plans that meet their unique needs.

Key Responsibilities
  • Provide high-quality care to patients with mental health problems, working closely with our multidisciplinary team to deliver evidence-based interventions.
  • Design and deliver recovery-focused mental health interventions and groups, tailored to meet the unique needs of each patient.
  • Work collaboratively with patients, families, and carers to develop personalized care plans that promote recovery and well-being.
  • Contribute to the development of our Community Mental Health Service, sharing your expertise and ideas to improve patient outcomes.
Requirements
  • Relevant professional qualification in mental health nursing, occupational therapy, or social work, with current professional registration.
  • Experience of working in a mental health setting, with a strong understanding of principles of assessment, risk assessment, and management.
  • Ability to work collaboratively as part of a multidisciplinary team, with excellent communication and interpersonal skills.
