About the Role

We are seeking a qualified Mental Health Practitioner to join our Integrated Care Team within primary care, supporting individuals with long-term physical and mental health issues to maximize their potential and well-being.

The successful candidate will have a minimum of two years' experience working within community mental health services and possess an ability to work autonomously within a primary care community setting.

The role requires a full and valid driving licence and access to a vehicle for work purposes due to the community element of this role.

Main Responsibilities
  • Facilitate mental health/cognitive assessments and formulate diagnosis, treatment/post diagnostic plans with the supervising Consultant Psychiatrist.
  • Employ a person-centred, psychosocial approach to practice, enabling independence and avoiding hospital admissions where possible.
  • Develop excellent risk assessment, communication, and documentation skills, as well as a willingness to problem solve and signpost to other services, where appropriate.
Requirements
  • Professional Qualification in Mental Health Nursing or Occupational Therapy.
  • Non-medical prescribing.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

Salary: £48,270 to £54,931 a year (Incl. 5% Fringe HCAS) per annum, pro rata.
