About the Role

We are seeking a highly motivated and experienced Community Mental Health Nurse to join our team in North East Hampshire. As a key member of our multidisciplinary team, you will be responsible for assessing new referrals and working with an allocated caseload of service users, providing low-level psychological interventions and coordinating their care under the Care Programme Approach.

You will need to demonstrate excellent communication skills and be able to provide clear information around health promotion and psychoeducation to people who use our services. Additionally, you will be required to engage in medicines management and physical health monitoring, as well as provide educational input and supervision to nursing students and junior colleagues.

Key Responsibilities
  • Assess new referrals and work with an allocated caseload of service users
  • Provide low-level psychological interventions and coordinate care under the Care Programme Approach
  • Engage in medicines management and physical health monitoring
  • Provide educational input and supervision to nursing students and junior colleagues

Requirements
  • 5 years experience working with adults with Mental Health Problems (including training), or additional relevant post registration training
  • Experience working in either in primary or secondary community mental health care
  • Able to carry out supervision
  • NMC Registered Nurse - RMN/RN (Mental Health)
  • Successful completion of Mentorship course
