Key Responsibilities- Manage a caseload of patients with a variety of mental health conditions, providing assessments, interventions, and education.
- Collaborate with other healthcare professionals to ensure seamless care and support for patients.
- Work closely with other PCN-based roles to address the potential range of biopsychosocial needs of patients.
- Complete comprehensive risk assessments and develop strategies to manage risk indicators.
- Provide support, information, and training to family members, carers, and other professionals as needed.
- Hold a professional qualification at diploma or degree level in mental health nursing or equivalent occupational therapist qualification, with valid registration with the governing body.
- Have a clear understanding of child protection policy and procedures, with a commitment to the safeguarding of children and vulnerable adults.
- Have experience of working in both primary care and community settings, with a strong understanding of mental health problems and treatment options.
- Be able to communicate complex and sensitive information effectively with people at all levels, and have the ability to identify and assess/manage risk when working with individuals.
We offer a competitive salary of £44,962 to £52,809 per annum, depending on experience, and a range of benefits, including ongoing professional development opportunities and a supportive working environment.
