We are seeking a highly skilled and experienced Care Coordinator to join our team in Dudley Wide PCNs.

About the Role

This is an exciting opportunity to work across the whole PCN, supporting our patient population and their personalised care requirements.

The successful candidate will need a flexible approach in the delivery of the PCN's aims for patient centred care.

The role requires movement around all of Practices so access to a car is essential.

Main Responsibilities
  1. Identify individuals' care and support needs and explore options to meet these into a single personalised care plan in collaboration with primary care teams.
  2. Provide coordination and navigation for elderly people and their carers across health and care services, working closely with social prescribing link workers, occupational therapists, and dietitians.
  3. Refer individuals to the wider integrated care team to help manage their needs.
  4. Conduct face-to-face annual assessment reviews of patients' social, physical, and mental health needs.
  5. Participate in integrated care teams to ensure the co-ordination of care.
  6. Support practices with Cancer screening programmes and assist the PCN Cancer Lead with data collection and auditing.
  7. Maintain accurate time sheets and other data on the service.
  8. Work to agreed standards, policies, and procedures, and be fully conversant with emergency procedures.
Requirements
  • 5 GCSEs Grade C or above, including Maths and English (or equivalent).
  • NVQ Level 3 Business Administration or similar administrative qualification.
Knowledge and Experience
  • Knowledge of public health issues in the local area.
  • Knowledge of health promotion strategies.
  • IT skills, including EMIS, Office, and Outlook.
  • Experience working in Primary Care environments.
  • Partnership/collaborative working experience and building relationships with professionals across various organisations.
  • Working knowledge and experience of using IT systems.
  • Ability to extract salient points from large amounts of information, analyse data, and communicate findings.
  • Experience as a Care Coordinator and working with professionals in healthcare settings.
  • Knowledge/familiarity with medical terminology.
