About the Role

We are seeking a highly motivated and experienced Family Support Practitioner to join our Early Help team on a fixed-term basis. As a key member of our team, you will play a vital role in providing proactive, integrated, and innovative support to families in need.

Key Responsibilities:

  • Conduct comprehensive assessments to identify the needs of families and develop effective support plans.
  • Provide direct support to families, including leading team around the family meetings and working with complex cases.
  • Collaborate with internal and external stakeholders to ensure a coordinated approach to supporting families.
  • Stay up-to-date with the latest research and best practices in family support and apply this knowledge to inform your work.
