Family Support Practitioner - Early Intervention Specialist

About the Role

We are seeking a highly skilled and dedicated Family Support Practitioner to join our Early Help team. As an Early Intervention Specialist, you will play a vital role in providing proactive, integrated, and innovative support to families most in need.

Key Responsibilities

  • Complete assessments and develop tailored support plans to meet the unique needs of each family.
  • Lead team around the family meetings to ensure effective collaboration and coordination.
  • Understand and address the complexities of mental health, drug and alcohol use, domestic abuse, and other factors impacting families.
