Mental Health Practitioner

We are seeking an exceptional Mental Health Practitioner to join our Leaving Care Team (16+). As a key member of our team, you will work collaboratively with young people's advisors, CLA health, and adults' services to support young people's physical, emotional, and mental health needs in preparation for transition into adulthood and independence.
The successful candidate will be pivotal in our service development to improve the outcome for care leavers by effectively addressing mental health needs. You will be responsible for consulting, assessing, providing intervention, and signposting or referring onto appropriate services, as well as providing consultations to other allied professionals.
