Mental Health Practitioner in Leaving Care Team

We are seeking an exceptional Mental Health Practitioner to join our Leaving Care Team, working with young people aged 16+ to support their physical, emotional, and mental health needs in preparation for transition into adulthood and independence.
The successful candidate will work collaboratively with young people's advisors, CLA health, and adults' services to provide consultations, assessments, interventions, and signposting or referrals to appropriate services. They will also provide professional supervision and guidance to a Wellbeing Support Worker.
This is an exciting opportunity to make a positive impact on the lives of care leavers and contribute to the development of our service to improve outcomes for this vulnerable group.
