We are seeking an exceptional Mental Health Practitioner to join our Leaving Care Team. The successful candidate will work collaboratively with young people's advisors, CLA health, and adults' services to support young people's physical, emotional, and mental health needs in preparation for transition into adulthood and independence.

The role will involve consulting, assessing, providing intervention, and signposting or referring onto appropriate services. Additionally, the successful candidate will provide professional supervision and guidance for a Wellbeing Support Worker.

The successful candidate will be pivotal in our service development to improve the outcome for care leavers by effectively addressing mental health needs.

Key Responsibilities:

  • Consult and assess young people's mental health needs
  • Provide intervention and support to young people
  • Signpost or refer onto appropriate services
  • Provide professional supervision and guidance to a Wellbeing Support Worker
