About the Role

We are seeking an exceptional Mental Health Practitioner to join our Leaving Care Team, working with young people aged 16+ to support their physical, emotional, and mental health needs. The successful candidate will work collaboratively with our team to provide professional supervision and guidance to a Wellbeing Support Worker.

This is an exciting opportunity to work in a dynamic and innovative team, contributing to the development of our service to improve outcomes for care leavers. You will have the opportunity to work with a diverse range of young people, providing tailored support to meet their individual needs.

Key Responsibilities:

  • Work collaboratively with the Leaving Care Team to provide mental health support to young people aged 16+
  • Provide professional supervision and guidance to a Wellbeing Support Worker
  • Contribute to the development of our service to improve outcomes for care leavers
  • Work with a diverse range of young people, providing tailored support to meet their individual needs
