About the Role

We are seeking a skilled and experienced Lead Nurse to join our team at East Suffolk and North Essex NHS Foundation Trust. In this role, you will be responsible for coordinating the NHS CHC process, including conducting nursing assessments and ensuring that the Care Domains within the national framework for NHS continuing healthcare are applied against each continuing healthcare assessment.

Main Responsibilities
  • You will support and facilitate the safe discharge of patients from Ipswich Hospital and community hospitals in Ipswich and East Suffolk.
  • Work collaboratively with other clinical services, social care, voluntary services, care sector, community services, and across Alliance partners.
  • Provide trusted assessor roles to care home providers.
Requirements
  • Evidence of knowledge of hospital discharge and community policy.
  • Ability to demonstrate the importance of integrated working.
