About the Role

We are seeking an experienced Advanced Clinical Practitioner to join our team at Wentworth 1 Primary Care Network. As a key member of our primary care network, you will play a vital role in delivering high-quality patient care and contributing to the development of our network.

Key Responsibilities
  • Work autonomously and be accountable for your own professional actions, utilizing advanced clinical skills and expert knowledge to deliver holistic care to patients.
  • Assess undifferentiated patients with a range of acute, non-urgent, and chronic medical conditions, utilizing your expertise to formulate differential diagnoses and develop management plans.
  • Undertake pre-booked emergency clinics, face-to-face consultations, telephone, or video consultations, ensuring timely and effective patient care.
  • Work collaboratively with our multidisciplinary team to promote integrated and seamless pathways of care, ensuring the best possible outcomes for our patients.
  • Monitor the effectiveness of your clinical practice through quality assurance strategies, such as audit and peer review, to continuously improve patient care.
Requirements
  • Registered Adult Nurse, Paramedic, or Pharmacist with the relevant governing body.
  • Independent non-medical prescriber with a minimum of 2 modules at Level 7, including assessment of clinical examination skills.
  • Experience of working in primary care, with a strong understanding of complex and common conditions in adults and children.
  • Consolidated independent prescribing experience and use of examination skills in practice.
  • Experience of telephone triage and working in a GP practice.
