  • Collaboration and Communication: Work effectively with a diverse range of stakeholders, including service users, families, carers, and professionals, to ensure seamless support and care.
  • Relationship Building: Develop and maintain positive relationships with service users, their families, and colleagues, fostering a culture of trust, respect, and open communication.
  • Care and Support Planning: Contribute to the development and implementation of care and support plans, ensuring that each individual's unique needs and goals are met.
Requirements
  • Person-Centred Approach: Demonstrate a person-centred approach, focusing on the strengths, abilities, and preferences of each individual.
  • Experience and Skills: Possess experience working with adults with learning disabilities, promoting independence and well-being, and excellent communication and collaboration skills.
  • IT and Recording: Have good IT skills and accurate information recording abilities, ensuring that all relevant data is recorded and maintained.
Working Arrangements
  • Shift Pattern: Work a shift pattern, including bank holidays and every other weekend, to respond to the demands of the service.
  • Rotating Shifts: Rotate through early, late, and sleep-in shifts, ensuring that the service is always staffed and ready to meet the needs of service users.
