Job Summary

We are seeking an experienced Advanced Care Coordination Specialist to join our team at University Hospitals of North Midlands NHS Trust. The ideal candidate will have experience in healthcare transformation and organisational development, with a strong understanding of case management and bed management.

The successful candidate will work closely with the Virtual Ward Matrons to deliver case management, bed management, and acuity management of patients suitable for Supported Early Discharge. They will also be responsible for ensuring the correct utilisation of Virtual Ward beds and identifying suitable patients for Virtual Ward care.

Key responsibilities include:

  • Coordinating care pathways for patients transitioning from acute hospitals to virtual wards.
  • Working with hospital staff to educate and empower them to identify suitable patients for Virtual Ward care.
  • Developing and implementing strategies to improve patient outcomes and reduce length of stay in acute hospitals.

We offer a competitive salary of £41,526 - £49,683 per annum, depending on experience.
