Key Responsibilities

You will be responsible for:

  • Conducting assessments to identify individual needs and preferences
  • Developing and implementing personalized care plans to promote independence and community integration
  • Collaborating with colleagues to ensure seamless delivery of support services

A satisfactory DBS check and subsequent registration for the DBS Update Service are required. We offer a competitive salary of £32,000-£40,000 per annum, depending on experience, and opportunities for professional growth and development.
