About the Role

Key Responsibilities
  1. Assessment and Treatment: Conduct comprehensive assessments of individuals with learning disabilities, identifying their health needs and developing personalized care plans.
  2. Advice and Support: Provide expert advice and support to other staff members, ensuring that they have the necessary skills and knowledge to deliver effective care.
  3. Complex Information Management: Interpret and compare complex information from assessments, making informed decisions about individual care plans.
  4. Leadership and Supervision: Provide leadership, operational support, and supervision to Learning Disability Community Nursing staff, promoting a culture of excellence and continuous improvement.
  5. Clinical Supervision: Offer professional support and clinical supervision to relevant practitioners within the discipline, ensuring that they have the necessary skills and knowledge to deliver high-quality care.
  6. Reporting and Documentation: Develop and maintain reports, programs, and guidelines reflecting specialist knowledge, to support and address complex health needs.
  7. Accessible Information: Provide information in an accessible format, ensuring that individuals with learning disabilities and their families can understand and engage with their care plans.
  8. Service User Involvement: Encourage and support individuals with learning disabilities and their families to make informed decisions about their care, developing their healthcare knowledge and skills.
  9. Expertise and Advisory Role: Develop and maintain expertise in specific clinical areas, providing an advisory role to the team and contributing to the development of clinical interventions and recommendations.
  10. Training and Development: Identify, plan, and deliver relevant training to other professionals, carers, and organizations, ensuring the effective implementation of clinical interventions and recommendations.
  11. Service Development: Promote a positive approach to change, facilitating the development of the SALDHS service and contributing to the improvement of care outcomes.
  12. Team Coordination: Coordinate the multi-professional team closely with the Clinical and Operational Lead (CLOP) for SALDHS, ensuring effective communication and collaboration.
