Community Health Nurse for Early Intervention and Family Support

We are seeking a skilled and compassionate Community Public Health Nurse to join our health visiting team in Tunbridge Wells District. As a key member of our team, you will work with children and families in Paddock Wood and Cranbrook, providing essential support and care.
Your primary focus will be on delivering the Health Child Programme, working in accordance with national and local strategy/guidance. You will undertake universal and targeted assessments, mandated well-being reviews, and packages of care, ensuring the best possible outcomes for children and families.
As a Community Public Health Nurse, you will be responsible for contributing to antenatal and postnatal care programmes, completing HCP mandated reviews, packages of care, and safeguarding. You will work closely with the team to ensure the delivery of high-quality care and services.
This is a part-time opportunity, with a minimum of 22 hours per week. You will be based in the community, working from your base and visiting clients in their homes. We are committed to providing a flexible and supportive work environment, allowing you to balance your work and personal life.
Key Responsibilities- Deliver the Health Child Programme, working in accordance with national and local strategy/guidance
- Undertake universal and targeted assessments, mandated well-being reviews, and packages of care
- Contribute to antenatal and postnatal care programmes
- Complete HCP mandated reviews, packages of care, and safeguarding
- Registered Healthcare Professional with relevant Professional body - NMC
- Higher education modules in: Level 6 Early Intervention in Public Health, Level 6 Child Development and Assessment
- Experience of recent community/acute healthcare gained through education or work experience placement or previous employment
