Job Title: Mental Health Practitioner

We are seeking a highly skilled and experienced Mental Health Practitioner to join our team at South West London and St Georges Mental Health NHS Trust.

About the Role

This is an exciting opportunity to work in a dynamic and innovative team, providing specialist rehabilitation support to adults with complex mental health needs. As a Mental Health Practitioner, you will work closely with individuals, families, and carers to develop and implement personalized recovery plans, promoting independence and well-being.

Key Responsibilities
  • Provide specialist clinical input and advice to support colleagues within the Community Rehabilitation Service and external agencies.
  • Undertake time-limited treatment specific work with clients, coordinating the input of others and evaluating its effectiveness.
  • Develop and implement systematic plans for care, including medication management and adherence strategies.
  • Work as an autonomous practitioner, providing expert assessments and developing, implementing, and evaluating care/treatment interventions.
  • Lead the promotion of evidence-based practice for people with co-occurring mental health, alcohol, and/or drug issues.
  • Provide expert advice to clinical staff on assessment, planning, implementation, and evaluation of care/treatment packages.
  • Provide clinical supervision to staff of all disciplines and grades.
  • Work closely with acute hospitals to support referral and engagement to Hep C treatment for service users within mental health services.
  • Manage a caseload of service users affected by drug and alcohol use and severe and enduring mental health needs.
Requirements
  • Degree or equivalent experience in a relevant clinical subject.
  • Current professional registration in a relevant clinical subject.
  • Clinical Supervision training.
  • Evidence of continuing professional and personal development.
  • Experience of working in a multi-professional team.
  • Understanding of the skills needed to supervise members of a multi-disciplinary team.
  • Experience of working with people with co-occurring mental health, alcohol, and drug problems.
  • Experience of clinical risk assessment and management of vulnerable client groups.
