Community Care Coordinator

Creative Support is a national organization providing innovative solutions to meet the diverse needs of individuals with learning disabilities. We are currently recruiting a Community Care Coordinator to join our team in Morecambe. As a key member of our supported living service, you will be responsible for developing and maintaining warm, trusting relationships with our service users. Your primary focus will be on supporting individuals to achieve their goals and live valued, fulfilling lifestyles. You will also be responsible for planning and coordinating rotas to ensure that our users receive the best possible care.
