Care Coordinator for Dementia Care

Role Overview
">Tripod Nursing is seeking a qualified Registered Mental Health Nurse (RMN) to join our team as a specialist in memory assessment and dementia care. As a key member of our multidisciplinary team, you will work closely with healthcare professionals to provide high-quality care and support to individuals with dementia.
Key Responsibilities
">- Conduct comprehensive assessments to identify individual needs and develop personalized care plans.
- Work collaboratively with colleagues to ensure seamless care delivery and effective communication.
- Provide expert guidance and mentorship to staff and trainees to enhance their knowledge and skills.
- Identify and address risk factors to ensure patient safety and well-being.
- Support the team manager in delivering effective leadership and oversight of care processes.
Position Details
">- Location: Morecambe
- Work Pattern: Monday to Friday, 37.5 hours per week
- Contract Duration: 3 Months
- Rate: £25.00 per hour
- Pay Type: Umbrella or PAYE available
Requirements
">- Qualified RMN with current NMC registration.
- Specialist knowledge and experience in dementia care and memory assessment services.
- Strong assessment, risk management, and care planning skills.
