Key Responsibilities

  • To provide leadership and strategic direction to the community team in delivering clinical services and implementing policies and procedures.
  • To work closely with other departments within the hospice to ensure seamless integration of services and effective communication.
  • To develop and maintain effective working relationships with multi-disciplinary teams and external partners to enhance patient care and service delivery.
  • To undertake holistic assessments, planning, and delivery of specialist palliative and supportive care in accordance with hospice policy, procedures, and standards.
  • To establish and maintain accurate records of patient interactions, treatments, and outcomes, ensuring adherence to data protection and confidentiality principles.
  • To participate in quality improvement initiatives, contributing to the development of new services, processes, and protocols to enhance patient experience and satisfaction.

Requirements

  • A registered nurse with extensive experience in palliative care, preferably in a community setting.
  • Proven leadership and management skills, with the ability to motivate and inspire team members.
  • Excellent communication and interpersonal skills, with the capacity to build strong relationships with colleagues, patients, and families.
  • Strong analytical and problem-solving skills, with the ability to navigate complex healthcare systems and make informed decisions.
  • Ability to work independently and as part of a team, prioritizing tasks and managing time effectively.
  • Familiarity with relevant legislation, policies, and guidelines governing palliative care services, including safeguarding, consent, and confidentiality.

Salary: £60,000 - £70,000 per annum (dependent on experience).
