About the Role

We are seeking a highly skilled and dedicated Senior Support Worker to join our Mental Health Intensive Support Service team in East Cheshire. As a key member of our multidisciplinary team, you will play a vital role in providing intensive support to individuals with complex mental health needs, helping them to recover and regain the skills and confidence to live successfully in the community.

Key Responsibilities
  • Support the multidisciplinary team in providing high-quality levels of care to patients, promoting their recovery and independence.
  • Work closely with the primary worker and occupational therapist to ensure effective and adaptive patient care, identifying relapse indicators and promoting independence.
  • Develop and implement individualized care plans, ensuring that patients engage in agreed care plans and participate in local community services.
  • Provide training and support to care staff and third-party organizations to ensure seamless care delivery in the community.
  • Contribute to the prevention of crisis and hospital admission, demonstrating excellent risk management, adaptive care planning, and communication skills.
Requirements
  • Recent relevant experience of working with individuals with mental health issues towards recovery.
  • Experience of working within a mental health community setting, rehab setting, or equivalent.
  • NVQ Level 3 in Health & Social Care or equivalent.
  • Understanding of and commitment to principles of community rehabilitation and recovery models.
  • Experience of producing and leading teaching and learning in presentational sessions or groups.
