Senior Support Worker

We are seeking a highly motivated and experienced Senior Support Worker to join our Mental Health Intensive Support Service team in East Cheshire. As a Senior Support Worker, you will play a vital role in providing intensive support to individuals with complex mental health needs, helping them to recover and regain the skills and confidence to live successfully in the community.
Key Responsibilities- Provide high-quality levels of care and support to individuals with mental health issues, working closely with the Multi-Disciplinary Team (MDT) to develop and implement personalized care plans.
- Assist individuals in engaging with local community services and promoting daily living skills, with a focus on recovery and independence.
- Develop and maintain effective therapeutic relationships with individuals, contributing to the planning and monitoring of interventions and treatments under the supervision of the MDT.
- Participate in training and education of care staff and third-party organizations to ensure seamless support in the community.
- Contribute to the prevention of crisis and hospital admission, demonstrating excellent risk management, adaptive care planning, and communication skills.

Person Specification- Recent relevant experience of working to support and integrate people with mental health issues towards recovery.
- Experience of working within a Mental Health community setting, rehab setting, or equivalent.
- NVQ Level 3 in Health & Social Care or Equivalent.
