About the Role

We are seeking a highly skilled and dedicated Senior Support Worker to join our Mental Health Intensive Support Service team in East Cheshire. As a key member of our multidisciplinary team, you will play a vital role in providing intensive support to individuals with complex mental health needs, helping them to recover and regain the skills and confidence to live successfully in the community.

Key Responsibilities
  • Support the multidisciplinary team in providing intensive support to a small cohort of patients, promoting high-quality levels of care and assisting individuals to live fulfilled and independent lives.
  • Work alongside the Primary Worker and Occupational Therapist to ensure effective and adaptive patient care, with a strong focus on recovery, identifying relapse indicators, and promoting independence.
  • Develop and maintain effective therapeutic relationships with patients, contributing to the planning and monitoring of interventions and treatments under the supervision of the multidisciplinary team.
  • Participate in training and education of care staff and third-party organizations to support patients in the community, such as Supported Living staff or Care Agency staff.
  • Contribute to the proactive management of risk, adaptive care planning, and communication across a range of scenarios to prevent crisis and hospital admission.
