Mental Health Support Specialist

We are seeking a highly skilled and dedicated Senior Support Worker to join our Mental Health Intensive Support Service team in East Cheshire. As a key member of our multidisciplinary team, you will play a vital role in providing intensive support to individuals with complex mental health needs, helping them to recover and regain the skills and confidence to live successfully in the community.
Key Responsibilities- Support the multidisciplinary team in providing intensive support to a small cohort of patients, promoting high-quality levels of care and assisting individuals to live fulfilled and independent lives.
- Work closely with the primary worker and occupational therapist to ensure effective and adaptive patient care, with a strong focus on recovery, identifying relapse indicators, and promoting independence.
- Develop and implement individualized care plans, ensuring all patients engage in agreed care plans and assist in encouraging inclusion in local community services to promote daily living skills.
- Contribute to the training of care staff and third-party organizations that support patients in the community, ensuring a seamless and effective support network.
- Demonstrate excellent skills in risk management, adaptive care planning, and communication skills across a range of scenarios, proactively pre-empting crisis and preventing hospital admission.

We are looking for individuals who possess the following skills and experience:
- Recent relevant experience of working to support and integrate people with mental health issues towards recovery.
- Experience of working within a mental health community setting, rehab setting, or equivalent.
- NVQ Level 3 in Health & Social Care or Equivalent.
- Understanding of and commitment to principles of the Community Rehabilitation and Recovery Models.
- Experience of producing and leading teaching and learning in presentational sessions/in groups.
- Experience of working with clients in recovery from serious mental health issues.
