About the Job

We are looking for an experienced and compassionate Nurse to join our Out-of-Hours Community Nursing team in South Staffordshire.

The successful candidate will have a strong commitment to delivering high-quality patient-centered care, working independently and as part of a collaborative team.

As a Band 5 Nurse, you will be responsible for assessing patient needs, developing and implementing personalized care plans, and ensuring timely interventions to prevent hospital admissions.

You will work closely with patients, families, and other healthcare professionals to provide comprehensive care and support.

A valid driving license and access to a car are required due to the geographical area covered by the role.

Job Requirements:

  • Current NMC registered general nurse (degree in nursing)
  • UK Car and driving license
  • Relevant experience in community nursing
