About the Position: We are seeking a skilled and compassionate Home Care Professional to join our team at City and County Healthcare Group Ltd. As a Complex Care Assistant, you will provide exceptional care and support to individuals with complex needs, working closely with our multidisciplinary team to deliver personalized care plans.

Job Description: Your primary responsibility will be to deliver high-quality care and support to individuals with complex needs, including:

  • Providing hands-on care and support to individuals with complex needs
  • Developing and implementing personalized care plans in collaboration with our multidisciplinary team
  • Delivering training and education to colleagues and clients on various aspects of care
  • Maintaining accurate records and reporting any changes or concerns to the relevant authorities

Salary and Benefits: This full-time role offers a competitive salary of £12.50 per hour, plus a range of benefits including maternity/paternity leave, pension scheme, paid annual leave, refer a friend scheme, cycle-to-work scheme, and enhanced DBS check.
