Primary Care Team Member

We are looking for a dedicated Clinical Practitioner to join our team at Practice Plus Group. As a key member of our primary care team, you will play a vital role in delivering high-quality healthcare services to patients in a secure environment.
The successful candidate will have a strong background in healthcare, including a current professional qualification and evidence of continued professional development. A knowledge of healthcare policies and best practice is also essential.
In this role, you will be responsible for delivering primary care services to patients, including providing medical assessments, developing care plans, and working with other healthcare professionals to ensure the best possible outcomes.
