About the Role

We are seeking a Support Worker to join our Young People Community Support service in the South Glos area. As a Support Worker, you will play a vital role in supporting young people to live the lives they want to lead.

Key Responsibilities

  • Empower young people to develop daily living skills and access their communities for socialisation, employment, training, or friendships.
  • Contribute to, develop, and implement individual care plans to meet the unique needs of each young person.
  • Communicate effectively and professionally with young people, their family members, and external professionals to ensure seamless support.
  • Maintain the health needs of the young people we support, including personal care and medication management.

Requirements

  • Good communication skills and a positive, patient approach.
  • Ability to build professional relationships and work well as part of a team.
  • Confident problem-solving and creative thinking skills.
  • Resilience and respect for people's differences.
