We are looking for an experienced Adult Mental Health Practitioner to join our dedicated team at Kings Norton Hospital.

About the Role

The successful candidate will be responsible for ensuring the care, welfare, and safety of all patients are maintained at the highest possible standard. This will involve creating individualised care plans, liaising with external suppliers and healthcare providers, and monitoring the performance of support workers.

Key Responsibilities

  • Delivering high-quality patient care and maintaining a safe clinical environment
  • Developing and implementing effective care plans tailored to individual patient needs
  • Liaising with multidisciplinary teams and external partners to ensure seamless service delivery
  • Contributing towards multidisciplinary working, risk assessment management, and on-going clinical improvement
