Job Description

This Adult Social Care Practitioner role involves delivering adult social care services, conducting person-centered, holistic assessments, and providing interventions that focus on strengths-based and multi-agency approaches. You will manage a varied caseload, perform risk assessments, and undertake legal and statutory duties.

Your Key Responsibilities
  • Deliver high-quality adult social care services, conducting person-centered assessments and providing interventions
  • Liaise with health services to coordinate discharges from the hospital and prevent admissions
  • Develop and implement practice standards across the service, contributing to achieving performance outcomes

The ideal candidate will possess excellent communication and analytic skills, allowing them to negotiate and influence outcomes effectively and work collaboratively with diverse agencies. A passion for delivering person-centered support and improving outcomes drives commitment to maintaining high professional standards.

Requirements and Qualifications

We require a recognized qualification and current HCPC registration. The successful candidate will also have experience in conducting holistic assessments, care planning, and coordinating with multi-disciplinary teams in social care or health settings.

Salary and Benefits
