Job Description- About This Role:
- A dynamic and passionate Leaving Care Navigator is required to join our Sefton team, focusing on delivering high-quality support services to care leavers.
As a key member of our team, you will work closely with care leavers, their families, and multi-agency teams to develop and implement personalized Pathway Plans.
Key Duties and Responsibilities- Main Job Functions:
Provide expert guidance and support to care leavers on financial management, housing, education, employment, and community participation.
Collaborate with care leavers, their families, and multi-disciplinary teams to ensure comprehensive support.
Ensure accurate and up-to-date records are maintained, adhering to data protection regulations and complying with national standards.
Foster strong relationships with local authorities, partner agencies, and community organizations to facilitate seamless service delivery.
Participate in staff development activities to enhance professional expertise and contribute to the improvement of service quality.
The estimated salary range for this position is £24,000 - £39,000 per annum, based on experience and qualifications.
Required Qualifications and Skills- A Level 3 qualification related to working with children and young people.
- In-depth knowledge of child and young person development, family dynamics, and the challenges faced by care leavers.
- Excellent communication and interpersonal skills, with the ability to establish trust with vulnerable individuals.
- Familiarity with relevant legislation and planning tools for Looked After Children and Care Leavers.
