Main Responsibilities

The successful candidate will be responsible for providing leadership, clinical excellence, and innovation in palliative and end-of-life care within the Luton community nursing service. This will involve working in partnership with other providers to deliver a seamless service across all care settings.

Key Duties
  1. Provide holistic health assessments of patients with complex palliative and end-of-life care needs in community settings.
  2. Plan, coordinate, deliver, and evaluate the provision of high-quality specialist palliative and end-of-life care within the community and across the primary and secondary care interface.
  3. Make clinical decisions independently and seek senior guidance where necessary in reviewing, monitoring, and managing patients.
  4. Assess, plan, and evaluate care for patients, their families, and carers.
  5. Provide opportunistic informal education to patients, their families, and health professionals.
  6. Work as part of a multidisciplinary team acting as patient advocate for specialist palliative and end-of-life nursing advice and care.
  7. Participate in the Palliative and End-of-Life Care educational programme to deliver evidence-based education to community-based staff.
  8. Actively participate in regular clinical supervision.
  9. Develop and sustain effective working partnerships with local clinicians, other specialist palliative care providers, managers, and team colleagues.
  10. Understand and ensure good practice in managing patient information between different care settings and teams.
Requirements
  • Currently working in a B6 role or above.
  • Significant and recent clinical knowledge of palliative and end-of-life care.
  • Evidence of teaching/training experience.
  • Audit/evaluation of service/project.
  • Current knowledge of national policies and drivers relevant to specialist palliative and end-of-life care.
Desirable
  • Community nursing experience.
Skills
  • Ability to demonstrate good IT skills.
  • Excellent communication and interpersonal skills - written and verbal with evidence of basic counselling skills.
  • Ability to be flexible working across organisations and work with a range of professionals.
  • Effective teaching and presentation skills, using different methods, training, and providing written reports.
