Job Title: Children's Community Nurse Practitioner - Paediatric Care Specialist

Job Summary: We are seeking a skilled Paediatric Nurse Manager to join our team at Leaders in Care. As a Paediatric Nurse Manager, you will be responsible for providing face-to-face support to children and their families, creating personalised care plans, and training and supporting them with their clinical skills and activities.

Key Responsibilities:

* Design and deliver tailored care plans specific to an individual client's needs
* Provide ongoing monitoring, support, and assessment of client's physical and physiological needs within your caseload
* Deliver nursing interventions, including the management of medicine delivery
* Work closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care

Requirements:

* RSCN with a minimum of 6 months in the UK and an active NMC PIN
* Up to date experience of tracheostomy care and ventilators within the last 2 years
* Suitable for Band 5 / Band 6 Nurses with evidence of effective leadership and management of HCAs
* Full UK Driving Licence desirable but not essential
