### Job Title: Advanced Nurse Practitioner

### Job Summary

We are seeking an experienced Paediatric Nurse to join our team as a Paediatric Nurse Manager. As a Paediatric Nurse Manager, you will be responsible for providing face-to-face support to children and their families, creating personalised care plans, and working closely with a core team of carers.

### Key Responsibilities

* Design and deliver tailored care plans specific to individual client needs
* Effectively manage the multidisciplinary team through strong communication and teamwork
* Provide ongoing monitoring, support, and assessment of client physical and physiological needs
* Deliver nursing interventions, including medicine management
* Build and maintain relationships with client families
* Work closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care

### Requirements

* Registered Sick Children's Nurse (RSCN) with a minimum of 6 months' experience in the UK and an active NMC PIN
* Supportive management style that brings out the best in others
* Up-to-date experience of tracheostomy care and ventilators within the last 2 years
* Advanced communication skills verbally and written to liaise effectively with patients, families, and the MDT
* Suitable for Band 5/Band 6 Nurses with evidence of effective leadership and management of HCAs
* Full UK Driving Licence desirable but not essential

### Benefits

* Up to £50,000 per annum DOE
* 2 days working from home per week/3 days in the community
* Monday to Friday (9-5) hours
* 33 days holiday
