Job Opportunity

We are seeking a highly skilled and dedicated professional to join our Community Learning Disability and Autism Team as an Adult Advanced Practitioner. This is a rewarding opportunity to provide professional support, leadership, and line management to promote the independence, inclusion, and well-being of adults with a learning disability.

The successful candidate will work collaboratively with a multi-disciplinary team, including Community Learning Disability Nurses, Social Workers, and Social Care Assessors, to deliver a range of services including care management, transitions, safeguarding, and mental health support. You will be responsible for promoting personalised care pathways, ensuring choice and control for eligible individuals, and influencing and shaping the local environment to promote social inclusion and accessibility.

To be successful in this role, you will need a degree or equivalent Social Work qualification, registration with a professional body, and a commitment to professional development. You will also need to be able to work effectively with others, build strong relationships, and demonstrate a passion for promoting the rights and dignity of individuals with a learning disability.
