Job Opportunity

We are seeking a highly skilled and dedicated professional to join our Community Learning Disability and Autism Team as an Adult Advanced Practitioner.

Job Summary

This is a challenging and rewarding role that requires a strong understanding of social care principles and practices. As an Adult Advanced Practitioner, you will provide professional support, leadership, and line management to the team, promoting the independence, inclusion, and well-being of adults with a learning disability.

Key Responsibilities
  • Provide leadership on risk and safeguarding matters, ensuring the safety and well-being of service users.
  • Develop and implement personalised care pathways, promoting choice and control for eligible individuals.
  • Work collaboratively with the multi-disciplinary team to deliver high-quality care and support services.
  • Contribute to the development and implementation of strengths-based approach projects, promoting social inclusion and accessibility.
  • Provide professional social work advice and guidance to colleagues and stakeholders.
Requirements

To be successful in this role, you will need:

  • A degree or equivalent Social Work qualification, registered with a professional body.
  • Demonstrable commitment to professional development and continuous learning.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • A strong understanding of social care principles and practices, including risk and safeguarding.
